Triglyceride–Glucose Index and TyG-BMI for Identifying Advanced Cardiovascular–Kidney–Metabolic Syndrome: A Cross-Sectional Study

Abstract

This study aimed to investigate the associations of TyG and TyG-BMI with the presence of advanced CKM syndrome, and to compare their discriminatory performance for advanced CKM Syndrome. This cross-sectional study included 2, 060 patients with CKM, including 1, 216 patients with early CKM, stages 1 to 2, and 844 patients with advanced CKM syndrome, stages 3 to 4. Patients with advanced CKM syndrome had significantly higher levels of total cholesterol, low-density lipoprotein cholesterol, triglycerides, fasting blood glucose, TyG, and TyG-BMI than those with early CKM, whereas highdensity lipoprotein cholesterol levels were significantly lower, all P < 0.001. Multivariable logistic regression analysis showed that, compared with the lowest quartile (Q1), the odds of advanced CKM syndrome were significantly increased in the Q3 group of the TyG (OR = 1.894, 95%CI: 1.270-2.825) and the Q4 group (OR = 6.503, 95%CI: 4.310-9.813). Similarly, compared with Q1, the odds of advanced CKM syndrome were higher in the Q3 group of the TyG-BMI (OR = 1.976, 95%CI: 1.327-2.944) and the Q4 group (OR = 6.926, 95%CI: 4.583-10.466). Receiver operating characteristic curve analysis showed that the areas under the curves for the TyG and the TyG-BMI in discriminating advanced CKM syndrome were 0.678 and 0.699, respectively. In conclusion, patients with advanced CKM syndrome exhibited more pronounced insulin resistance-related metabolic abnormalities. TyG and TyG-BMI were independently associated with advanced CKM syndrome. Both indices demonstrated moderate discriminatory ability and may serve as simple screening tools; however, prospective studies are needed to validate their predictive value.
